* AUDITION APPOINTMENTS ARE OPTIONAL

1) Online registration is closed. You will need to register and sign up for Audition Appointments at the door the day of the conference. There are still Audition Appointments available.

2) Two appointments allowed per paid participant. Consult the Appointment guidelines before completing this form. Requests that do not conform to guidelines will be returned unscheduled. Please be advised that none of your appointments will be scheduled until the conference and appointment fees are received.

3) You may fill in the fields on this screen and then print the filled in form or click here to print a blank form, and fill in by hand. Please bring your Audition Appointment form to the Registration Table the day of the conferences
